These two types of jobs refer to which parts are replaced. Axle back jobs are the simpler of the two, because fewer parts are replaced. An axle back exhaust replacement job usually only includes the muffler and a few pipes, which are the parts located at or behind the axle. The muffler is designed to come off, so you can simply pop it off and screw the new fuel efficient muffler in place. Cat back jobs, however, include everything from the catalytic converter to the back of the vehicle. These parts are more complicated to replace, as they usually are not designed to be removed. You will probably have to cut them off. This also means you will need to take extra steps to secure the new part in place. While doing a cat back exhaust job is more expensive and requires more know-how, it does also provide the biggest boost to your fuel efficiency.
It is also possible to replace some deep exhaust parts, such as the exhaust manifold, but these parts take even more work to replace. In fact, you may have to remove the engine entirely in order to access the manifold.
